If I only buy one thing, what should it be?

A Dutch oven. Soups, chili, braises, baked pasta and even bread all come out of the same pot, so it earns its cupboard space quickly.

Do I need both a cast iron skillet and a Dutch oven?

They do different jobs. The skillet is for searing and one pan dinners, the Dutch oven for anything with liquid that cooks slowly.

Is an immersion blender worth it just for soup?

If you make soup regularly, yes. It blends right in the pot, which saves both moving hot liquid to a blender and washing the blender afterwards.
